Shoulder pain has several causes. Shockwave is not a general solution for a painful shoulder. The approach depends inter alia on which tendon is involved and whether calcification is present.

A diagnosis before a device choice

One randomised study in 86 adults with long-term symptoms of the rotator cuff tendons without calcification found better pain and function results in focused shockwave plus EMTT than in focused shockwave plus placebo EMTT, with follow-up up to 24 weeks. Both groups also received physiotherapy. This is a limited positive finding. The JOSPT guideline of 2025 does not recommend shockwave in case of uncalcified symptoms from the rotator cuff tendons. The combination is therefore not self-evident standard treatment; the consideration requires discussion of research, guidelines and alternatives.

In case of calcification in a tendon, nuance is required

Focused shockwave is used in calcified tendinopathy of the shoulder. Directives differ in their assessment of their operation. The JOSPT guideline of 2025 mentions shockwave in calcification as a possibility. The UK guideline organisation NICE does not consider the evidence to be sufficient and advises this application only in research. That is why we discuss the uncertainty and do not present this as a proven standard solution.

Discuss other options

The consideration is part of a broader discussion about diagnosis, practice therapy and other treatment options. There is no automatic shockwave treatment based on shoulder pain alone.
