What is happening in this design?
The orientation help only uses your choices temporarily to create the view. They are not stored in the browser, not sent to a server and not used for diagnosis. When leaving the orientation aid, the choices will be deleted.
Your appointment request
The appointment form asks for your name, date of birth, email address, telephone number, symptom description, zip code and address. After your consent and shipment, Shockwave Clinics and Spine Clinics will receive this information to contact you and schedule an appointment. If online requests are not yet available, the form will state this and your data will not be sent.
Where do your records go?
Your request is sent through our website host Vercel and email service Resend to info@spine-clinics.nl. Connections to these services are encrypted. The email contains your submitted details; the email service also processes the message to deliver it. The website does not create its own patient database or save your request in browser storage. Your data is used by the team handling your request, not for marketing. Do not send identity numbers or medical documents, and keep your description brief.
Temporary technical data
Temporary counters in server memory help prevent abuse and expire within one hour. Resend uses a sending reference for up to 24 hours to prevent duplicate sends. Hosting and email services also process technical logs and delivery data. These periods do not describe how long the practice retains the received email. A request does not automatically book an appointment or subscribe you to marketing.
Limited website statistics
We use Google Analytics only if you allow statistics. We measure visits to general page groups, contact clicks and successfully sent requests. We do not send names, contact details, descriptions of symptoms, search queries or orientation answers. Medical pages are measured under general categories without their specific topic. Advertising personalisation and automatic form tracking are disabled. Your choice is remembered on this device for 180 days. You can change or withdraw consent via Cookie settings in the footer. Google processes the measurement data; without consent, Google Analytics is not loaded and the form still works.

When you open an external source or Spine Clinics, you go to another website. There you have your own privacy and cookie policy.
To withdraw consent or request access, correction or deletion, email info@spine-clinics.nl. Do not include medical details in your request. The team will discuss which data has been processed, how long it is needed and how your request can be handled.
